I’ll always remember my visits to San Francisco and passing groups of older adults in the parks, practicing Tai Chi. But what is Tai Chi, why do people practice it, and can anyone do it?

Tai Chi was originally a deadly art, guarded by a few families and used for killing. Traditionalists believe that it is important for students of this ancient art to remember its roots, because the techniques of relaxation and breath control were developed for the express purpose of injuring the opponent in an efficient, scientific manner.

Today, of course, we no longer need to practice this martial art for the purpose of killing our enemy. However, some say that now we can use this practice to fight the enemy of fatigue, stress, overwork or lack of understanding of oneself and one’s body. Daily practice of Tai Chi promotes mental clarity and a healthy body, assists with balance and helps the circulation of the blood.

Someone doing Tai Chi (pronounced “tie chee”) will move slowly and gently, while breathing and meditating. This is why it is sometimes called "moving meditation". Many practitioners believe that Tai Chi helps the flow throughout the body of a proposed vital energy called qi (pronounced "chee," it means "air," "puff," or "power"). What is the Medical Home Concept?

While the medical home concept has its origins in pediatric care, the concept has expanded as the general healthcare system has contemplated the shift from a focus on episodic acute care to a focus on managing the health of defined populations, especially those living with chronic health conditions.

Several seminal commentaries influenced thinking about how team-based care might improve clinical care and achieve optimal population health, establishing the foundation for a more detailed conceptualization of the medical home:

> The Chronic Care Model, a structured approach for clinical improvement through team based care supported by an organizational and information technology infrastructure, which is the basis for the Bureau of Primary Health Care's (BP) Health Disparities Collaborative.
> The Institute of Medicine's (IOM) first Quality Chasm report which articulated Six Aims and Ten Rules to guide the redesign of healthcare, including the importance of team-based care. This roadmap for improving quality in the healthcare system stated that healthcare should be safe, effective, patient-centered, timely, efficient, and equitable. The Chronic Care Model, Health Disparities Collaborative and Quality Chasm Aims and Rules are described in Appendices A and B.

Building on this foundation, the American Academy of Family Physicians, American Academy of Pediatrics, American College of Physicians, and American Osteopathic Association released their Joint Principles of the Patient-Centered Medical Home in 2007.

> Personal physician - each patient has an ongoing relationship with a personal physician trained to provide first contact, continuous, and comprehensive care.
> Physician directed medical practice - the personal physician leads a team of individuals at the practice level who collectively take responsibility for the ongoing care of patients.
> Whole person orientation - the personal physician is responsible for providing for all the patient's healthcare needs or taking responsibility for appropriately arranging care with other qualified professionals. This includes care for all stages of life: acute care, chronic care, preventive services, and end of life care.
> Care is coordinated and/or integrated across all elements of the complex healthcare system (e.g., subspecialty care, hospitals, home health agencies, nursing homes) and the patient's community (e.g., family, public and private community based services). Care is facilitated by registries, information technology, health information exchange, and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.
> Quality and safety are hallmarks of the medical home.
> Enhanced access to care is available through systems such as open scheduling, expanded hours, and new options for communication between patients, their personal physician, and practice staff.
> Payment appropriately recognizes the added value provided to patients who have a patient-centered medical home.

Barr recently summarized the rationale for the patient-centered medical home, pointing to the unwarranted variation in our nation's delivery of healthcare and the lack of relationship between what is spent and the quality of the services that are delivered. He also notes that, while research suggests a robust primary care system is a major characteristic of an efficient and high-quality healthcare system, the U.S. primary care system is uncertain, perhaps close to collapse.

About the Wisconsin Health Information Exchange


WHIE operates within the National Institute of Medical Informatics (NIMI), a nonprofit organization dedicated to research and development of advanced healthcare information networking. The mission of WHIE is to provide a system where diverse stakeholders collaborate to securely exchange confidential health information between authorized users. We envision a world where authorized health professionals and patients access information securely, when and where it is needed, to improve the safety, quality and efficiency of health care and public health. The core of this model is structured around continuity of care conce pts, placing the patient at the center of care.
WHIE is a collaborative organization overseen by a broad based Board of Advisors and staff of three. WHIE invites interested organizations and individuals to become members of WHIE. Members include hospitals, outpatient clinical practices, skilled nursing facilities, healthcare professional groups, laboratories, imaging centers, pharmacies, private health plans, ERISA plans, government plans, public health agencies, researchers or other data users, patient advocate organizations, professional associations, and corporate or philanthropic organizations. Members of WHIE are eligible to run for election to the Advisory Board.
WHIE has been funded by Wisconsin Department of Health Services, through a Medicaid Transformation Grant, together with contributions from the participating health systems in Milwaukee County and payers, including Humana and the Business Health Care Group. WHIE operates on the Microsoft Amalga platform and was the first HIE operating on this platform.
Since our early planning sessions in 2004, WHIE has sought to influence healthcare delivery. Today health care providers are beginning to see change initiated by the Health Information Technology for Economic and Clinical Health (HITECH) Act. Providers are assessing the impact of incentives from the Centers for Medicare and Medicaid Services to adopt electronic prescribing systems. With broad implementation, the secure exchange of health information using National Health Information Network standards, services and policies, will lead to regional and interstate connectivity and help improve the quality and efficiency of healthcare for all Americans.
On the state level health information exchange planning is initiated under the Wisconsin Relay of Electronic Data (WIRED) for Health project, funded through the Office National Coordinator for Health Information Technology (ONC).

Health Information Exchange (HIE) refers to the process of reliable and interoperable electronic health-related information sharing conducted in a manner that protects the confidentiality, privacy, and security of the information. The development of widespread HIEs is quickly becoming a reality. Health Information Organizations (HIOs) are the organizations that oversee HIE.  For HIOs to function, they must have the capability to employ nationally recognized standards to enable interoperability, security and confidentiality, and to ensure authorization of those who access the information.   
 
The HIE implementation challenge will be to create a standardized interoperable model that is patient centric, trusted, longitudinal, scalable, sustainable, and reliable.  HIM principles will be critical to the success of HIEs and the nationwide health information network. HIEs, enabled by technology, are expected to improve the quality of care and patient safety and reduce healthcare costs.

HIM professionals are among the experts stakeholders needed for health information exchange.  Healthcare workflow, patient privacy rights, state and federal disclosure laws, secondary data, and data integrity issues (including duplicate patient records) are all part of the HIM professional’s body of knowledge. This knowledge base uniquely positions the HIM professional to navigate the barriers to Health Information Exchange (HIE) and facilitate the definition of key workflows and processes. It also qualifies the HIM professional to assume a leadership role in an HIE.

AHIMA is taking an active role in standards setting efforts for HIE and has convened a Practice Council of Experts appointed by the Board of Directors to provide guidance to the Association concerning HIE. Through development of privacy and security policies regarding methods for accessing the HIE system, provisioning, authorizing, and authenticating users, and auditing access,HIM professionals have the opportunity to engage with colleagues and other stakeholders in defining functional business processes in the various systems sharing health data. These processes should be based on best-practice data quality principles and attributes that result in improved quality of care and patient safety. ealth information exchange (HIE) is defined as the mobilization of healthcare information electronically across organizations within a region, community or hospital system.
HIE provides the capability to electronically move clinical information among disparate health care information systems while maintaining the meaning of the information being exchanged. The goal of HIE is to facilitate access to and retrieval of clinical data to provide safer, more timely, efficient, effective, equitable, patient-centered care. HIE is also useful to Public Health authorities to assist in analyses of the health of the population.
HIE systems facilitate physicians and clinicians meeting high standards of patient care through electronic participation in a patient'scontinuity of care with multiple providers. Secondary health care provider benefits include reduced expenses associated with: duplicate tests, time involved in recovering missing patient information, paper, ink and associated office machinery, manual printing, scanning and faxing of documents, the physical mailing of entire patient charts, and manual phone communication to verify delivery of traditional communications, referrals and test results. According to an internal study at Sushoo Health Information Exchange, a single-clinician practice spends $17,160/year associated with the current method of exchanging patients' health information.
Formal organizations are now emerging to provide both form and function for health information exchange efforts, both on independent and governmental/regional levels. These organizations are, in many cases, enabled and supported financially by statewide health information exchange grants from the Office of the National Coordinator for Health Information Technology. These grants were legislated into the HITECH components of the American Reinvestment and Recovery Act in 2009.[1] The latter organizations (often called Regional Health Information Organizations, or RHIOs) are ordinarily geographically defined entities which develop and manage a set of contractual conventions and terms, arrange for the means of electronic exchange of information, and develop and maintain HIE standards.[2]
In the United States, federal and state regulations regarding HIEs and HIT (health information technology) are still being defined. Federal regulations such as "Meaningful Use" legislation[3] as well as the implementation of some state governments of state-sponsored HIEs (such as the North Carolina HIE[4]) in addition to fluctuating health care regulations among the states are rapidly changing the face of this relatively new industry. HIEs and RHIOs continue to struggle to achieve self-sustainability and the vast majority remain tied to Federal, State, or Independent grant funding in order to remain operational; with some exceptions such as the Indiana HIE
